The woman fallen which was talking to you is named Sift and had no apparent item on her whatsoever.
The big one is indeed called Doo-Hel and had a giant sword.)

(When something you planned was supposed to have no effects whatsoever, but it affects the player, because he's in a truck...)
You guessed right, creepers are coming out of the elevator, but something else happens.
Cubes start to rise from the ground, creating multiple elevations.
Your truck falls over gently on the side. There's no impact, but the wheels are not touching the ground and you're still in the truck.
Waluigi: "Wah! What is going on?"
The creepers seem to be able to jump one height of cube at a time. 5 of them are going towards you, 5 towards Mike and 5 towards Waluigi.
