The first time, you did attack twice, as evident by the "Kress attacks again!"
The second time, you didn't attack twice because your first hit killed him.
As for avoiding, the calculations came out to about a 63% hit rate. RNGGGGGGGGGGG

I messed up the calculations...
Okay, if Kress attacks again he'll do proper damage. I blame Just for messing up his character's stats earlier anyways.
