Serena casts Stabilize on the elf. Cantrips are second nature to her. Then she concentrates, tunes out everything around, ignores the heat and noise and the elemental spraying water everywhere... And casts a Healing Hex on the elf.
The elf gasps and snaps awake. "Ah... Ah!" She yells something in Elvish - a swear, most likely. Serena's glad when she starts speaking in Common. "H-help me..."
"Don't worry, I'm here to help. Are you hurt?"
"There's. A. Shelf. On. Me."
"Is anything broken or anything?" Serena asks. "I need to know if it's safe to try to drag you out."
"I... I don't think so..."
...Sosa should be on his way, and there's no immediate danger. But Serena can tell her Aura of Purity won't last much longer. Not that that's her only way to deal with the smoke, but it's the only way she can deal with the smoke in a large area.
(A) Try to make a sling from some blankets and drag the case away with the broom
(B) Grab and pull! With the elemental to help! You, too, elf!
(C) Have the water elemental blast her with water
(D) Wait for Sosa to get here

Serena goes to the bookcase and grabs it. She signals to the elemental to join in. "Alright, Miss. When I say 'go,' try to push up against the shelf as best you can. Ready? Go!"
"Hnnngaah!" The elf starts screaming in pain. "Stop, stop...!"
Serena freezes. "What's wrong?"
"My leg..."
Serena looks underneath the shelf and notices that there's some sort of metallic rod stabbed through her leg. "What? How did this happen?"
"I was trying to save... heirloom... Ngh..."
Serena fights back the urge to call her an idiot and remain professional. This makes things harder.
Sosa comes running through the door with a few happy yips. He bounces over to Serena's side and rubs against her. One of the ogres comes wandering in behind him. Serena shakes her head and looks again. She's not hallucinating. "Huh? What are you doing here!?"
The ogre shrugs. "You say 'follow fox.' I follow fox."
"Where are the other ogres?"
"Outside."
Serena groans. ...Wait, maybe this is good?
(A) Use Delay Pain (Level 2 remaining: 3) on the elf so she won't feel it, and have the ogre pull off the shelf.
(B) Use Squeeze (Level 2 remaining: 3) to help the elf slip out
(C) Pull the damn thing out of her leg and use Cure Moderate Wounds (Level 2 remaining: 3) on the injury
(D) Use Enlarge Person (Level 1 remaining: 3) to make the elf bigger

I'm good with A or C. D is a particularly bad idea, making her larger while there's something stabbed into her is only going to make the injury even worse.

"Huh? What... did you do to me?" she asks.
"Alright, Mr. Ogre, help me lift the bookcase!" Serena says.
"W-w-wait!" the elf whimpers, but it is too late. The ogre nearly throws it into the wall. Serena grabs her and pulls her clear, just in case. "...Huh? It doesn't hurt..."
Serena winks. "Magic."
"But, my..." The elf looks down at her leg and sees the metal rod-thing still stuck in her leg. She screams. Not in pain, just panic.
The water elemental fades. Aura of purity won't last much longer.
(A) Let her scream and just drag her out
(B) Try talking to her calmly
(C) SHAKE HER AND YELL
(D) Use Sleep (Level 1 remaining: 3) to put her to sleep
(E) Use... Unnatural Lust (Level 2 remaining: 2) to make her want to climb into the ogre's arms?
(F) WHIP OF SPIDERS (Level 2 remaining: 2) MAKES EVERYTHING BETTER

A. Our mission isn't to keep her from screaming, it's to keep her from dying. Besides, she's not kicking around and stuff.
